Reading a pattern - buttonholes

I am trying to knit buttonholes on two rows and I am having trouble understanding the following line:

Rib 3, cast off 2sts, (rib 7, cast off 2) 3 times, rib 2.

I have 38 sts to start with and I am working in 2x2 rib. Each time I knit the first row where I need to cast off I end up with the wrong number of stitches at the end, generally too many. I think I am casting off the wrong stitches, can anyone help me understand the instructions please?

Rachel, does the pattern say how many stitches you should have at the end of the first row? You should end up with 30 as you are casting off 8 in total and then you will be casting the same number on in the next row.
